Approximately 30% of adverse events leading to hospitalization are due to medication errors, interactions or side effects.Medicare beneficiaries with multiple chronic illnesses see on average 13 different physicians, fill 50 prescriptions each year, account for 76% of all hospitalizations and are 100 times more likely to have a preventable hospitalization than those with no chronic illnesses.Data suggests that only 33-50% of patients with chronic conditions adhere to their prescribed medication therapies.In 2012, 73% of physician visits resulted in at least one prescription medication.Discharge orders from hospitals can be confusing and inconsistent and difficult to navigate. Studies show that adding a pharmacist to a patients overall healthcare team helps prevent adverse events.Rania Shenoda, PharmD, joined Sansum Clinic to start their first Prescription Navigator program, that assists patients and doctors in getting their meds right. Ms. Shenoda earned her undergraduate degree in Psychobiology from the University of California, Los Angeles and her Doctor of Pharmacy degree from the University of Southern California.
