In 2021, THREE words almost cost me my life. Two words were added to my medical chart that changed the way I was examined, and the one word that was left OFF my medical chart, delayed life saving care. Our medical charts have a lot of responsibility, power, and influence. Everyone that participates in our health care system is reliant on medical charts and the details it contains. It helps with collaboration and continuity of care, but it can also save a patient’s life in an emergency. However, the one thing most patients don’t know about their medical record is that it is THEIRS. Various providers may notate and contribute to the documentation it contains, but the information, the history, the knowledge, the data…all of it belongs to the patient. This is why it’s so important to know how to manage your medical chart.Medical charts also give patients the reasonable expectation that their record will be comprehensive, accurate, unbiased, and confidential. Within these expectations are two rights that are important and well protected: access and accuracy.
