For example, when patients go from acute to post-acute care, it’s essential that the patient arrives at the right facility as they continue to heal. Ashley had first-hand experience of what can go wrong when her sister who had serious health care struggles ended up with the wrong level of care. Ashley wants to be a part of the solution, and Re-Assist makes this process easier for everyone. RE-Assist is designed to become the ultimate referral assistance management app for patients in transitions of care, case management, and vendors to offer educated choices to improve quality of life.
