"How Community Health Workers fit in Team Base Care" is presented by Bryan Carcamo, BS, CHW from KC CARE Health Center and Treva Smith, CHW-I from Community Care Network of Kansas.Community Health Workers (CHWs) are non-medical public health workers who connect communities to health care and social service providers. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. Bryan Carcamo and Treva Smith will explain how CHWs are essential in helping communities have better health outcomes.
