According to the United States Health and Human Services Office of the Inspector General, as much as 42 percent of evaluation and management (E/M) services billed to Medicare and Medicaid were incorrectly coded, including both up- and down-coding, while 19 percent lacked sufficient documentation. Increased scrutiny on public spending and decreased funding for public health service providers make it essential for practitioners working in public health family planning settings to accurately code patient visits and document the care provided.
