Client Feedback prompts (I'm using the form from my mastermind - just change it up for your own program and tweak to make your own):

Client name and title
How has the mastermind changed your thoughts and beliefs about yourself and your biz?
How has the mastermind changed how you show up, and what you do in your biz?
Basically… how has the mastermind changed you and your business?
What is your favourite part about the mastermind? (Or several.)
What is   now happening in your biz that wasn’t before… and/or what now feels possible   that didn’t before?
Why are you happy you followed the pull and said yes to this experience?
What would you say to someone thinking about joining the mastermind?
Anything   else you’d like me to know?
Have you attached a pic in your email that I can use on the sales page?
